Please read this instruction before you start filling the form.

1. Every line on the form is fill able. Make sure everything you entered is

correct and nothing is left out.

2. Once you are ready click on the Email button on the form. Your default email will

popup with the subject line already filled with the name of the form.

3. Enter bankbond@ethioconsulatela.org on the To: line and click send .

4. If for some reason your email is not configured to work this way.

5. Click on the save button and make sure it is saved in My document folder. Open

your email the way you always open and on the To: line enter the above email address, on

the subject line enter EEPCO BOND and attach the form as you always attach

documents when you send email.
